If you miss a weekly GLP-1 or tirzepatide injection, the correct action depends on which medicine you use and, in some cases, which country’s product information applies. Ozempic, Wegovy, Mounjaro and Zepbound do not all use an identical missed-dose rule.
For example, Ozempic generally allows a missed dose to be taken within five days, while Mounjaro and Zepbound use a four-day window. Current US and European instructions for a missed Wegovy injection are also expressed differently.
This guide compares the official missed-dose instructions for Ozempic, Wegovy, Mounjaro and Zepbound and explains what to do after multiple missed doses or a longer treatment interruption.
In brief
- Ozempic: a missed weekly dose can generally be taken within 5 days. After that, skip it.
- Mounjaro and Zepbound: a missed dose can generally be taken within 4 days (96 hours). After that, skip it.
- Wegovy: current instructions differ in wording between jurisdictions. US injection instructions use the time remaining before the next scheduled dose; current EU information uses a 5-day missed-dose window.
- Do not take an extra dose simply to make up for a skipped weekly dose.
- After missing multiple doses or having a longer interruption, contact your prescriber because restarting at a lower dose may sometimes be appropriate.
- Always follow the instructions supplied with your own medicine in your country when they differ from general online guidance.
GLP-1 missed-dose rules at a glance
| Medicine | Usual official missed-dose instruction | When to skip |
|---|---|---|
| Ozempic | Take the missed dose as soon as possible within 5 days | If more than 5 days have passed |
| Wegovy injection — US | If the next scheduled dose is more than 48 hours away, take the missed dose as soon as possible | If the next scheduled dose is less than 48 hours away |
| Wegovy injection — EU | Take the missed dose as soon as possible if 5 days or less have passed | If more than 5 days have passed |
| Mounjaro | Take the missed dose as soon as possible within 4 days (96 hours) | If more than 4 days have passed |
| Zepbound | Take the missed dose as soon as possible within 4 days (96 hours) | If more than 4 days have passed |
These rules apply to a single missed weekly injection. Missing several consecutive doses or restarting after a prolonged interruption can require different advice.
Product information can also change. Patients should use the current leaflet or prescribing information supplied for their own medicine and jurisdiction.
What should you do if you miss an Ozempic dose?
Current US and European Ozempic instructions use the same basic rule.
- If 5 days or less have passed since the dose should have been taken, administer the missed dose as soon as possible.
- If more than 5 days have passed, skip the missed dose.
- Take the following dose on the normal scheduled weekly day.
- Do not take an extra dose to compensate for a skipped dose.
For example, if your normal Ozempic day is Monday and you remember on Thursday, the missed dose is still within the five-day window and can generally be taken.
If the allowed window has already passed, the usual instruction is to skip that injection rather than trying to catch up immediately before the next scheduled dose.
What should you do if you miss a Mounjaro dose?
For Mounjaro, the missed-dose window is 4 days, or 96 hours.
- If no more than 4 days have passed, take the missed dose as soon as possible.
- If more than 4 days have passed, skip the missed dose.
- Take the next dose on the regularly scheduled day.
- Do not add an extra dose to compensate for one that has been skipped.
This rule is also covered in MediSearcher’s detailed tirzepatide dosing guide.
What should you do if you miss a Zepbound dose?
Zepbound contains the same active ingredient as Mounjaro — tirzepatide — and current US instructions use the same 4-day (96-hour) missed-dose window.
If the missed dose can be taken within four days, administer it as soon as possible and then return to the weekly schedule.
If more than four days have passed, skip that dose and resume treatment on the normal scheduled day.
What should you do if you miss a Wegovy dose?
Wegovy requires a little more care because current official instructions are not worded identically in the United States and European Union.
US Wegovy injection instructions
Current US prescribing information for Wegovy injection bases the decision on how long remains before the next scheduled weekly dose:
- If the next scheduled dose is more than 2 days (48 hours) away, administer the missed Wegovy dose as soon as possible.
- If the next scheduled dose is less than 2 days (48 hours) away, do not take the missed dose.
- Resume treatment on the regularly scheduled weekly day.
EU Wegovy instructions
Current European Wegovy product information instead tells patients:
- If 5 days or less have passed since the missed injection, use Wegovy as soon as possible.
- If more than 5 days have passed, skip the missed dose.
- Take the next injection as usual on the scheduled day.
- Do not use a double dose to compensate for a forgotten dose.
This is a good example of why patients should check the product information supplied with their own medicine rather than assuming that instructions found on a US website apply everywhere.
Why are the missed-dose rules different?
There is no universal missed-dose rule that applies to every medicine described casually as a “GLP-1.”
Semaglutide and tirzepatide are different active ingredients with different pharmacokinetic properties, and individual products have their own regulator-approved dosing instructions.
Even products containing the same active ingredient can have different indications, formulations or regulatory instructions.
The safest rule is therefore simple: follow the missed-dose instructions for the exact product you have been prescribed.
Can you take a GLP-1 dose one day late?
For the weekly injectable medicines covered in this guide, one day late will generally still fall within the official single-missed-dose window.
That does not mean every GLP-1 medicine has the same instructions, and patients should still check the leaflet supplied with their product.
After taking an allowed late dose, follow the product-specific instructions for returning to the normal weekly schedule.
Can you take Ozempic two days late?
Two days is within Ozempic’s five-day missed-dose window. The missed injection can generally be administered as soon as possible and the normal weekly schedule then resumed.
Can you take Mounjaro two days late?
Yes. Two days falls within Mounjaro’s four-day missed-dose window.
If a patient is repeatedly taking doses late rather than occasionally forgetting one, it may be useful to choose a different regular injection day that better fits their routine.
Can you take Mounjaro or Zepbound five days late?
Not under the standard single-missed-dose instructions.
Mounjaro and Zepbound use a four-day, or 96-hour, window. If more than four days have passed, the missed dose should generally be skipped and the next dose taken on the normal scheduled day.
Can you take Ozempic six days late?
The standard Ozempic instructions say to skip a missed dose once more than five days have passed.
A patient who is six days late would therefore normally skip that injection and resume on the regular scheduled day rather than taking two doses close together.
Should you double your next GLP-1 dose after missing one?
No.
A missed weekly dose should not be compensated for by taking twice the prescribed dose at the next injection.
Doing so could increase drug exposure and the risk of adverse effects, including gastrointestinal symptoms.
Follow the medicine’s missed-dose instructions and then return to the prescribed weekly dose.
What if you miss two or more Wegovy doses?
Current US Wegovy prescribing information specifically addresses this situation.
If two or more consecutive Wegovy injection doses are missed, treatment can be resumed as scheduled or, if necessary, Wegovy can be reinitiated using the dose-escalation schedule.
Re-escalating can help reduce gastrointestinal adverse effects associated with restarting treatment.
This means somebody who has been off Wegovy for several weeks should not automatically assume that the correct approach is to inject their previous maintenance dose without checking.
What if you miss several Ozempic doses?
The simple five-day rule is designed primarily for a single missed dose.
A longer interruption creates a different question because semaglutide levels fall progressively over time and tolerance to gastrointestinal effects may change.
If several weekly Ozempic doses have been missed, contact the prescribing clinician or pharmacist for advice about restarting rather than improvising a dose schedule.
What if you miss several Mounjaro or Zepbound doses?
The standard four-day rule likewise tells patients what to do with an individual missed tirzepatide dose.
After a longer treatment interruption, patients should ask the healthcare professional managing their treatment how to restart.
Tirzepatide is normally introduced gradually, beginning at a low dose and increasing in stages partly to improve gastrointestinal tolerability.
Restarting a previously tolerated high dose after a substantial break may therefore not be appropriate for every patient.
Can you change your Ozempic injection day?
Yes, provided the product-specific minimum interval between doses is respected.
For Ozempic, official product information allows the weekly administration day to be changed when necessary as long as sufficient time has passed between injections.
Once a new day is selected, treatment can continue once weekly on that day.
Can you change your Mounjaro or Zepbound injection day?
Yes.
For tirzepatide, the weekly administration day can be changed when necessary provided there are at least 3 days (72 hours) between two doses.
After changing the day, continue once-weekly treatment on the new schedule.
Is missing one GLP-1 dose dangerous?
For many patients, an isolated missed weekly dose is handled using the medicine’s standard missed-dose instructions and does not by itself constitute a medical emergency.
However, the clinical importance depends partly on why the medicine is being used.
Someone using Ozempic or Mounjaro for type 2 diabetes may experience worsening glucose control when treatment is interrupted, particularly if other diabetes treatment is insufficient.
Patients with diabetes should follow their existing glucose-monitoring plan and seek medical advice if their readings become substantially elevated or they develop concerning symptoms.
Will you get side effects after restarting?
Gastrointestinal adverse effects such as nausea, vomiting, diarrhoea and abdominal symptoms are common with semaglutide and tirzepatide, particularly during treatment initiation and dose escalation. MediSearcher’s guides to Wegovy side effects and Mounjaro side effects cover the common effects and more serious warning signs for each product.
After a sufficiently long break, restarting at a higher dose may make these effects harder to tolerate in some patients.
This is why multiple missed doses should be treated differently from being one or two days late.
If the interruption has lasted several weeks, ask the prescriber whether dose re-escalation is appropriate.
What if you vomit after a GLP-1 injection?
Weekly Ozempic, Wegovy, Mounjaro and Zepbound injections are administered under the skin rather than swallowed.
Vomiting after an injection therefore does not mean the injected dose has simply been expelled from the body.
Do not inject another dose merely because vomiting occurred.
Persistent or severe vomiting can cause dehydration and may require medical assessment, particularly if the patient cannot maintain adequate fluid intake.
What if you accidentally take two doses too close together?
Contact a healthcare professional, pharmacist, poison-information service or appropriate medical service for advice rather than waiting for symptoms to appear.
Do not attempt to correct the error by independently altering subsequent doses without guidance.
Excessive exposure can increase the likelihood of adverse effects, particularly gastrointestinal symptoms.
How can you avoid missing weekly injections?
Because these medicines are taken once weekly rather than every day, forgetting the injection day can be surprisingly easy.
Simple approaches can include:
- using a recurring phone or calendar reminder;
- choosing a consistent day linked to a regular weekly routine;
- recording each injection after it is taken;
- keeping the medicine in its correct storage location but using a visible reminder elsewhere; and
- planning ahead for travel or schedule changes.
Patients should not leave a refrigerated medicine somewhere inappropriate simply to make it more visible. Always follow the storage requirements for the specific product.
GLP-1 missed dose FAQ
How late can you take Ozempic?
A missed Ozempic dose can generally be taken within 5 days of the scheduled injection. If more than five days have passed, skip it and take the next dose on the regular scheduled day.
How late can you take Mounjaro?
Mounjaro can generally be taken within 4 days (96 hours) after a missed dose. After that, skip the missed injection and resume the regular schedule.
How late can you take Zepbound?
Zepbound uses the same basic four-day missed-dose rule as Mounjaro. If more than four days have passed, the missed dose should normally be skipped.
How late can you take Wegovy?
The answer depends on the applicable product information. Current EU instructions allow a missed injection to be taken when five days or less have passed. Current US Wegovy injection instructions say to take the missed dose when the next scheduled dose is more than 48 hours away and otherwise resume on the scheduled day.
Can I take my GLP-1 a day early?
Changing injection days is possible for some weekly products when the required minimum interval between doses is maintained. Check the instructions for the exact medicine rather than assuming the same timing rule applies to all GLP-1-based treatments.
What happens if I miss my GLP-1 for a week?
Do not automatically take an extra dose. Follow the missed-dose instructions for your medicine. If the interruption involves multiple scheduled doses, contact your prescriber or pharmacist about how to restart.
Should I restart at a lower dose after missing several weeks?
Sometimes dose re-escalation may be appropriate after a longer interruption, particularly to reduce gastrointestinal adverse effects. The correct restart dose depends on the medicine, previous dose, duration of the interruption and individual circumstances, so this should be confirmed with the prescribing clinician.
