What Happens When You Stop Ozempic, Wegovy or Mounjaro?

Stopping a GLP-1 or related incretin medicine does not usually cause a classic drug withdrawal syndrome, but the effects of treatment can diminish after the medicine is stopped. For people using semaglutide or tirzepatide for weight management, clinical trials show that weight regain is common after treatment ends. For people using these medicines for type 2 diabetes, blood glucose may also rise when the glucose-lowering effect is removed.

What happens after stopping depends on the medicine, why it was prescribed, how long it was used and the individual’s response to treatment.

This guide explains what clinical trials tell us about stopping Ozempic, Wegovy, Mounjaro and Zepbound, including weight regain, appetite, blood sugar and whether these medicines need to be tapered.

In brief

  • Weight regain is common after stopping GLP-1-based weight-management treatment.
  • In an extension of the STEP 1 trial, participants regained about two-thirds of the weight they had previously lost with semaglutide during the year after treatment stopped.
  • In SURMOUNT-4, people switched from tirzepatide to placebo regained substantial weight, while those who continued tirzepatide maintained and extended their weight loss.
  • Appetite and hunger may increase again as the medicine’s effects wear off.
  • For people taking Ozempic or Mounjaro for type 2 diabetes, stopping treatment can result in worsening blood glucose control.
  • Semaglutide and tirzepatide are not generally associated with a classic physical withdrawal syndrome.
  • Patients should not stop a prescribed medicine without discussing the reason and an alternative treatment plan with their healthcare professional.

What happens after you stop a GLP-1 medicine?

Medicines such as semaglutide and tirzepatide alter biological processes involved in appetite, food intake, glucose regulation and digestion while treatment is continuing.

Semaglutide, the active ingredient in Ozempic and Wegovy, acts as a glucagon-like peptide-1 (GLP-1) receptor agonist. Tirzepatide, the active ingredient in Mounjaro and Zepbound, acts at both GIP and GLP-1 receptors.

The medicines do not permanently reset those systems. As drug concentrations fall after treatment is stopped, their pharmacological effects gradually diminish.

Depending on why treatment was prescribed, this can mean:

  • increased appetite or hunger;
  • reduced feelings of fullness;
  • weight regain;
  • changes in blood glucose;
  • loss of some improvements in blood pressure, waist circumference or other cardiometabolic measures; and
  • improvement in treatment-related side effects such as nausea in people who experienced them.

The exact response varies substantially between individuals. Stopping treatment does not mean that every patient will regain all of the weight they lost or experience the same changes.

Do you regain weight after stopping Ozempic or Wegovy?

Weight regain is common after stopping semaglutide used for weight management.

One of the clearest sources of evidence comes from the extension of the STEP 1 trial.

In the original STEP 1 study, adults with overweight or obesity without diabetes received semaglutide 2.4 mg or placebo alongside lifestyle intervention for 68 weeks.

A subset of participants was then followed for a further year after treatment and structured lifestyle intervention had ended.

During the original treatment period, participants receiving semaglutide had lost an average of 17.3% of their body weight.

By week 120 — one year after semaglutide was stopped — they had regained an average of 11.6 percentage points.

That represented approximately two-thirds of their previous weight loss.

Despite the regain, average body weight in the semaglutide group remained about 5.6% below the original baseline at the end of follow-up.

Does everyone regain all the weight?

No.

Clinical-trial averages describe what happened across groups of participants, not what will happen to every individual.

Some people regain a large proportion of their lost weight, some regain considerably less, and individual circumstances can change after treatment ends.

Diet, physical activity, underlying biology, other medicines, health conditions and the amount of initial weight loss can all influence what happens afterwards.

It is therefore inaccurate to say that everyone who stops a GLP-1 medicine will automatically regain all of the weight they lost.

What the available evidence does show is that regain is common enough that maintaining weight loss after treatment withdrawal should be planned for rather than assumed.

What happens after stopping Mounjaro or Zepbound?

The strongest randomized withdrawal evidence for tirzepatide comes from the SURMOUNT-4 trial.

The study enrolled adults with obesity or overweight without diabetes.

Participants initially received tirzepatide for 36 weeks. During this period, they lost an average of 20.9% of their body weight.

Participants were then randomly assigned either to continue tirzepatide or switch to placebo for another 52 weeks.

From week 36 to week 88:

  • participants switched to placebo experienced an average 14.0% increase in body weight; while
  • participants who continued tirzepatide lost a further 5.5%.

At week 88, 89.5% of participants who continued tirzepatide maintained at least 80% of the weight they had initially lost, compared with 16.6% of those switched to placebo.

The results provide strong evidence that continued tirzepatide treatment helps maintain weight reduction and that substantial regain can occur after treatment is withdrawn.

Why can weight return after GLP-1 treatment stops?

Weight regain does not mean that the medicine has somehow damaged metabolism or that treatment has “failed.”

Obesity is a chronic condition influenced by biological, behavioural, environmental and genetic factors.

GLP-1-based medicines can reduce appetite and energy intake while they are being used. Tirzepatide and semaglutide can also improve several metabolic measures during treatment.

When the medicine is withdrawn, the pharmacological support that helped produce and maintain those changes is removed.

This is one reason modern obesity treatment increasingly treats weight management as a long-term process rather than a short course followed by an assumption that the biological drivers of weight gain have disappeared.

Does appetite come back after stopping a GLP-1?

It can.

Semaglutide and tirzepatide influence appetite and satiety, helping many patients feel full sooner or remain satisfied for longer after eating.

As the medicine leaves the body, these effects can diminish.

Some people therefore notice:

  • greater hunger;
  • less prolonged fullness after meals;
  • more interest in food;
  • larger portion sizes becoming easier to eat; or
  • more difficulty maintaining the reduced energy intake achieved during treatment.

The timing and intensity vary between patients and should not be interpreted as a personal failure or lack of willpower. Appetite regulation has an important biological component.

How quickly do Ozempic and Wegovy leave the body?

Semaglutide has a long elimination half-life of approximately one week.

Because elimination occurs gradually over multiple half-lives, semaglutide can remain in the circulation for several weeks after the final dose.

The effects therefore do not normally disappear immediately the day after the last injection.

This also helps explain why missing a single weekly dose is different from permanently stopping treatment.

How quickly does tirzepatide leave the body?

Tirzepatide has an elimination half-life of approximately five days.

Like semaglutide, it therefore leaves the body gradually rather than disappearing immediately after the final injection.

Appetite, glucose and gastrointestinal effects may consequently change progressively after treatment ends.

What happens to blood sugar after stopping Ozempic or Mounjaro?

This question is particularly important for people using these medicines for type 2 diabetes.

Ozempic and Mounjaro lower blood glucose while treatment is active. If treatment is stopped, that glucose-lowering effect is removed.

European Ozempic product information specifically warns patients not to stop treatment without speaking to their doctor because blood sugar levels may increase after Ozempic is stopped.

The effect in an individual patient depends on factors including their underlying diabetes, other glucose-lowering medicines, diet, body weight and pancreatic function.

A person using Ozempic or Mounjaro for diabetes should therefore have a treatment plan in place before discontinuation. Another medicine or a change to existing diabetes treatment may be required.

Do the health benefits disappear after stopping?

Some improvements can diminish when weight is regained or the medicine’s direct effects are removed.

In the STEP 1 semaglutide extension, improvements in several cardiometabolic measures moved back toward baseline during the year after treatment withdrawal.

More recent analysis of SURMOUNT-4 similarly found that greater weight regain after tirzepatide withdrawal was associated with greater reversal of improvements in measures including:

  • waist circumference;
  • blood pressure;
  • non-HDL cholesterol;
  • blood glucose measures; and
  • insulin resistance.

This does not mean that every benefit immediately disappears when treatment stops. The degree of change depends partly on how much weight is regained and the patient’s individual health profile.

Do you get withdrawal symptoms from Ozempic, Wegovy, Mounjaro or Zepbound?

Semaglutide and tirzepatide are not generally associated with a classic drug withdrawal syndrome of the kind seen with certain medicines that cause physiological dependence.

Instead, patients may notice the return of appetite, higher glucose levels or other features that the medicine had previously been controlling.

That distinction is important.

For example, feeling hungrier after stopping Wegovy is better understood as the medication’s appetite-suppressing effect wearing off rather than evidence of drug dependence.

Do you have to taper off Ozempic or Wegovy?

Official product information does not establish a standard dose-tapering schedule that every patient must follow when discontinuing semaglutide.

However, that does not mean patients should independently stop prescribed treatment.

The reason for stopping matters. For patients considering discontinuation because of adverse effects, MediSearcher’s guides to Ozempic side effects and Wegovy side effects cover common effects and symptoms that may require medical attention.

A clinician may need to consider:

  • whether treatment is being stopped because of side effects;
  • whether the medicine is being used for diabetes or weight management;
  • blood-glucose control;
  • other medicines;
  • the risk of weight regain;
  • pregnancy planning or another medical reason for discontinuation; and
  • whether another treatment should replace it.

The safest approach is therefore to agree a discontinuation plan with the healthcare professional responsible for treatment.

Do you have to taper off Mounjaro or Zepbound?

There is likewise no universal official tapering schedule requiring tirzepatide to be reduced through each available dose before treatment can end.

Patients should nevertheless discuss discontinuation with their prescriber, particularly when Mounjaro is being used to control type 2 diabetes or when another treatment will be needed.

Stopping because of a serious adverse reaction is also very different from electively ending otherwise well-tolerated long-term treatment. The Mounjaro side-effects guide covers common adverse effects and the warning signs that warrant medical attention.

Can you restart a GLP-1 after stopping?

In some circumstances, treatment can be restarted, but the appropriate dose depends on the medicine, the length of the interruption and individual tolerability.

Patients should not assume that they can automatically restart at the same high dose they were using before a long break.

Gradual dose escalation is used with semaglutide and tirzepatide partly to reduce gastrointestinal adverse effects. After a sufficiently long interruption, a clinician may decide that restarting at a lower dose and escalating again is more appropriate.

This is particularly relevant after multiple missed doses rather than a single late weekly injection.

What can help maintain weight after stopping?

No strategy can guarantee that weight will remain unchanged after treatment withdrawal.

However, a planned transition is preferable to simply stopping treatment without follow-up.

A long-term plan may include:

  • continued dietary support;
  • regular physical activity appropriate to the individual;
  • resistance exercise to support muscle mass;
  • regular weight and health monitoring;
  • management of sleep, stress and other factors affecting appetite;
  • review of medicines that may promote weight gain; and
  • consideration of another evidence-based weight-management treatment when appropriate.

People using medication for obesity should discuss long-term maintenance at the time treatment is prescribed rather than waiting until the final dose.

Is GLP-1 treatment supposed to be lifelong?

There is no single answer that applies to every patient.

Obesity and type 2 diabetes are chronic conditions, and some patients may benefit from long-term pharmacological treatment.

The withdrawal trials provide strong evidence that the benefits of semaglutide and tirzepatide are often easier to maintain while treatment continues.

However, whether a particular person should continue treatment long term depends on benefits, adverse effects, medical need, treatment availability, cost, patient preference and the availability of alternatives.

Patients should therefore avoid interpreting either “you must take it forever” or “you can stop once you reach your goal weight” as a universal rule.

Stopping GLP-1 treatment FAQ

Will I gain weight if I stop Ozempic?

Weight regain is common after semaglutide used for weight management is stopped, but the amount varies between individuals. In the STEP 1 extension, participants regained about two-thirds of their previous semaglutide-associated weight loss on average during the year after treatment ended.

Will I gain weight if I stop Mounjaro?

Substantial weight regain occurred when tirzepatide was withdrawn in the SURMOUNT-4 obesity trial. This does not mean every individual will regain the same amount, but it shows that continued treatment plays an important role in maintaining weight reduction for many patients.

Does appetite come back after stopping Ozempic?

It can. Semaglutide reduces appetite and increases satiety during treatment. As the medicine’s effect diminishes after stopping, some patients notice increased hunger or reduced fullness.

Does stopping Ozempic cause withdrawal?

Ozempic is not generally associated with a classic physical drug-withdrawal syndrome. Patients may instead experience the return of appetite or worsening of the condition that semaglutide was helping to control.

Can blood sugar rise after stopping Ozempic?

Yes. Ozempic lowers blood glucose in people with type 2 diabetes, and official European product information warns that blood sugar may increase after treatment is stopped.

Can you stop Wegovy once you reach your goal weight?

Reaching a target weight does not automatically mean Wegovy should be stopped. Clinical evidence shows that weight regain is common after semaglutide withdrawal. Long-term treatment and weight-maintenance planning should be discussed with the prescribing clinician.

Can you restart Ozempic or Mounjaro after stopping?

Restarting may be possible, but patients who have had a prolonged interruption should ask their prescriber what dose to use. Restarting immediately at a previously tolerated high dose may increase gastrointestinal side effects.
