Name the presentation, not just tirzepatide
Specify strength, single-dose or multidose device, quantity and scheduled doses. Four weekly doses represent 28 days, but a four-dose KwikPen is not the same device count as four single-dose pens. Match the authorization to the actual use
Weight-management and obstructive-sleep-apnea prescriptions can require different documentation. Have the clinician identify the true indication and ask the plan for its relevant criteria. An approved sleep-apnea indication does not by itself guarantee that a particular plan covers every Zepbound presentation.

Mounjaro offers do not establish Zepbound coverage
