Xigduo XR cost requires the full strength pair and daily tablet count. A current assistance change also matters: AZ&Me stopped accepting new Xigduo XR patients on May 1, 2026.
Do not equate once daily with one tablet
Give the pharmacy the complete dapagliflozin/metformin strength pair and the prescribed number of tablets each day. Ask it to confirm quantity and billed supply. A 30-tablet quote may not represent 30 days for every regimen, and a Farxiga-only quote omits metformin.
Use the combination’s coverage criteria
Ask the plan about Xigduo XR itself, the prescribed indication and any authorization. Current labeling limits the combination’s uses to type 2 diabetes because of metformin. Approval or coverage for dapagliflozin alone does not automatically establish the same result for Xigduo XR.
AZ&Me enrollment changed in 2026
AZ&Me says it stopped accepting new Xigduo XR patients on May 1, 2026. Existing participants should contact the program about their enrollment and renewal requirements. Older descriptions of open enrollment no longer apply to new applicants.

Checking other support options
AZ&Me patient assistance and commercial savings or self-pay programs have different rules. Ask specifically about current Xigduo XR participation, eligible insurance categories and limits. A Farxiga offer does not establish eligibility for Xigduo XR.
If separate ingredients are proposed
The prescriber must confirm an appropriate dapagliflozin and extended-release metformin regimen. Price both prescriptions for the same period and remove superseded combination orders. Include any separate monitoring and visit costs, and keep the clinician’s kidney, illness and procedure instructions intact during the change.

Questions and answers
Can a new patient assume AZ&Me covers Xigduo XR enrollment?
No. The program says new enrollment stopped on May 1, 2026.
Is a Farxiga savings offer valid for Xigduo XR?
Do not assume that. Product-specific terms need confirmation.
What is needed to compare separate ingredients?
A clinician-approved regimen, both exact products and the combined payment over the same supply period.
