Quote the intended Janumet formulation
For immediate-release Janumet, name 50/500 mg or 50/1000 mg and the prescribed tablet count. Twice-daily directions make a 60-tablet fill different from a 30-tablet fill. If the prescription says Janumet XR, request an XR-specific quote and directions rather than applying this immediate-release supply calculation.
Check the combination’s own coverage
Ask the plan whether Janumet is covered, whether authorization is required and whether the claim matches the intended quantity. Coverage of Januvia or metformin separately does not answer those questions. A new prescription or changed kidney function may require clinical review, not just a different billing code.
Applying for Merck assistance
Merck’s Janumet patient-assistance program has eligibility and application requirements. Insurance, residency and financial circumstances can affect eligibility. Check the current criteria and wait for confirmation of approval; it is not an automatic pharmacy coupon.
