| Medicine class | SGLT2 inhibitor / biguanide combination |

What the combination contains
Canagliflozin increases glucose excretion through the kidneys; metformin works through different glucose-regulating mechanisms, including reducing liver glucose production. The current US label includes glucose control in adults and children aged 10 years and older with type 2 diabetes. Specified cardiovascular and kidney benefits of canagliflozin apply to defined adult populations.

Immediate release and extended release
Both formulations have four strength pairs, ranging from 50/500 mg to 150/1,000 mg of canagliflozin/metformin. Immediate-release Invokamet is taken twice daily with meals; Invokamet XR is taken once daily with the morning meal. Follow the prescribed tablet count, which determines the daily amount of both ingredients.

Metformin brings a boxed warning
Metformin carries a boxed warning for lactic acidosis, a rare but serious complication. Severe kidney impairment is a contraindication. Acute illness, alcohol use and certain procedures can also affect safety. Invokana contains canagliflozin alone, so its instructions do not cover the additional precautions needed with metformin.

Canagliflozin brings additional precautions
Ketoacidosis, volume depletion, infections and lower-limb amputation risk are among the warnings. Foot problems and signs of dehydration or serious illness should be reported. A normal or moderately elevated glucose reading does not by itself rule out ketoacidosis.

Checking other diabetes medicines
Confirm with the prescriber whether Invokamet replaces separate canagliflozin or metformin tablets. Taking both the combination and one of its ingredients can unintentionally duplicate treatment. Bring the full medicine list or packs to the review.
